FEE REGULATIONS

for the Reservation of a Surgical Procedure Date

  1. These Regulations set out the rules governing the collection of an administrative fee (hereinafter referred to as the “Fee”) for the reservation of a plastic surgery procedure performed at the medical entity operated by Warsaw Aesthetic sp. z o.o., with its registered office in Warsaw /formerly: Chirurgia Plastyczna sp. z o.o./, hereinafter referred to as the “Healthcare Provider”.
  2. The administrative Fee is payable in connection with the reservation of a plastic surgery procedure and constitutes remuneration of the Healthcare Provider for the service of reserving and preparing the operating theatre for the surgical procedure, including ensuring the availability of the medical team and supporting personnel, securing access to medical equipment, and providing the instruments, materials and other medicinal products necessary for the planned surgical procedure.
  3. The Fee does not constitute a deposit or advance payment credited towards the price of the planned surgical procedure.
  4. The Fee is charged upon reservation of all plastic surgery procedures performed at the Healthcare Provider.
  5. Opłata wynosi 5.000 zł przy rezerwacji zabiegu Deep Plane Face Lift, 3.000 zł przy rezerwacji zabiegów z zakresu The Fee amounts to PLN 5,000 for the reservation of a Deep Plane Face Lift procedure, PLN 3,000 for the reservation of maxillofacial surgical procedures, and PLN 2,000 for the reservation of all other plastic surgery procedures.
  6. The Fee shall be paid upon reservation of the surgical procedure date at the Healthcare Provider, no later than within 3 days from the date of making the reservation of the surgical procedure.
  7. The Fee shall be paid by bank transfer to the bank account of the Healthcare Provider, in accordance with the details provided to the patient by the Healthcare Provider at the time of registration of the plastic surgery procedure.
  8. Failure to pay the Fee within 3 working days from the date of reservation of the plastic surgery procedure shall result in cancellation of the reserved surgical procedure date.
  9. The Fee shall be refunded by the Healthcare Provider in the event of a change or cancellation of the plastic surgery procedure date by the Healthcare Provider, or where the planned plastic surgery procedure cannot be performed due to the patient’s health condition /e.g. anaesthetic disqualification/.
  10. The Fee shall not be refundable in the event of cancellation of the plastic surgery procedure date by the patient or the patient’s failure to attend the Healthcare Provider on the scheduled date of the procedure.
  11. The patient shall be entitled to change the date of the planned plastic surgery procedure on one occasion only, provided that such change is requested no later than 30 days before the scheduled date of the procedure. In such case, the administrative Fee already paid shall be credited towards the administrative Fee payable for the surgical procedure rescheduled to the new date.
  12. Where the patient changes the date of the planned plastic surgery procedure less than 30 days before the scheduled date of the procedure, or where the patient requests any subsequent change of the procedure date, the administrative Fee shall not be refundable, and the patient shall be required to pay a new administrative Fee in connection with the reservation of the new surgical procedure date within the period specified in section 6 of these Regulations.
  13. Reservation of a procedure date shall be deemed equivalent to acceptance of these Regulations.
